New Patients

We welcome new patients to our practice. If a provider's schedule is full, we will have placed you on a waiting list to be contracted as soon as a spot is open. Walk-in appointments are also available on Monday, Wednesday and Thursday from 9am-3pm. How do I become a new patient?

Email

Once you fill out the New Patient Intake, please email it to info@salveohealth.org. We will contact you by phone within 24-48 hours of receiving your email. We are not able to respond to new patients via email due to HIPAA requirements.

You may also fax the new patient inquiry form to (770) 910-9197.

Cancellations

In order to accommodate all patients and respect our MD and Therapists' time, we require at least 24 hours notice for cancelled appointments. If 24 hours notice is not provided, there will be a $35 same-day cancellation fee applied. No-show appointments have a $75 fee applied. All payments must be made before seeing your provider. We appreciate your cooperation in making sure all patients who need to be seen are seen in a timely manner.

What to Bring

To speed up the process, please bring your completed form (although paperwork is also available upon arrival). Please bring your current insurance card, photo ID and payment for co-insurance, co-payment or deductibles. We look forward to seeing you soon!
